Questions Arising from the 'Pasha Bulker' Incident
Whilst the weight of the findings made by the ATSB in its recent report on the 'Pasha Bulker' incident relate to the conduct of the Master of the vessel, the report also raises some serious questions for the Newcastle Port Corporation ('NPC') and its Vessel Traffic Information Centre ('VTIC'). Indeed, 7 of the 13 'contributing safety factors' and 8 out of the 11 safety recommendations identified in the report are aimed at NPC. Many of these issues relate to NPC/VTIC being in the best position to issue timely weather warnings and directions to vessels such as the 'Pasha Bulker' but failing to do so, perhaps leading NPC/VTIC to ask themselves 'Should I have said something?'
